CPV charges only when someone actually views the ad, or watches it to a set point; CPM charges per thousand times it is served, whether or not it is watched. A seller states which its rate uses — the two are not comparable numbers.

Are video ads ever sold with a guaranteed placement?

Some are — a handful of sellers here quote a guaranteed daily or weekly rate rather than an auction, usually for a premium slot such as a homepage player. Most video inventory is still bought at auction.

Can I target a specific show or channel?

That depends on the seller: a direct sponsorship buy usually names the show; a programmatic buy targets an audience or a placement type across many videos, not one specific show, unless the seller offers a private marketplace deal.
