You buy from the publisher or its sales house and agree the placements, dates and price with them, usually on an insertion order.

ProgrammaticSoftware buys and delivers the ad, usually through a DSP. It covers deals at an agreed price as well as auctions, so programmatic is not the same as auction.
Ad networkA company that bundles ad spots from many publishers and sells them as its own. You buy from the network, not from the sites in it.
AffiliateThe publisher earns a commission on the sales or sign-ups it sends you, instead of charging for the ad itself.
